For employees, doctors do not have to issue Fitness to Work statememts for the seven days of sickness. For the first seven to eight days of sickness please ask your employer for a form SC2. If you require a fitness to work statement from your doctor for insurance purposes etc (or your employer), please ask the doctor for a private letter, for which a fee is payable; a receipt will be issued if requested. If you are still unwell after seven days, then you may ask your doctor for a free NHS Fitness to work statement.
If you need further statements you may not always need to see your GP if you have an ongoing condition, please telephone the surgery and give reception the dates that you need cover for. They will advise if you need to see a GP.
